  2. Difference in characteristics among the four diet groups were tested by analysis of variance for continuous variables and χ2-test for categorical variables and all characteristics statistically significantly differed among the four diet groups with P < 0.05
  3. Those with missing values were excluded: BMI (N = 3486), pack-years of smoking (N = 3382) and education (N = 2308)
